Grade IV refers to mesangial hyperplasia with crescent formation in 50 percent to 75 percent of glomeruli.

Grade 5 refers to mesangial hyperplasia with crescent formation in more than 75 percent of glomeruli.

Grade VI refers to pseudomembranous proliferative nephritis.
